What does a Community Link Worker do?

They work with staff at your GP practice. |

They work with people for around six to eight weeks. It might be more or less than this. |

They can talk with you to find out what important to you and what you would like to achieve. |

They can support you to put your plans into action. |

They can support you to meet your goals. |

Health Checks
You will be invited by your GP Practice every year, generally around your birthday.
It’s a chance for your Doctor or Nurse to get to know you and keep you healthy.
You can bring someone with you. We have guides and more information about how health checks help.
