What does a Community Link Worker do?

Easy read
Find easy information to help you understand health conditions, like Easy Read or videos.
Health Action Plan
Support you to meet your aims in your Health Action Plan.

Health Checks

Health checks are special appointments with a Doctor or Nurse each year.

You will be invited by your GP Practice every year, generally around your birthday.

It’s a chance for your Doctor or Nurse to get to know you and keep you healthy.

You can bring someone with you. We have guides and more information about how health checks help.
