What does a Community Link Worker do?

	They work with staff at your GP practice.
	They work with people for around six to eight weeks. It might be more or less than this.
	They can talk with you to find out what important to you and what you would like to achieve.
	They can support you to put your plans into action.
	They can support you to meet your goals.

A Community Link Worker can support you to:

	Join social groups.
	Find support groups.
	Join fitness classes.
	Meet new people.
	Find volunteering roles.
	Start a new course.
	Get support to gain employment.
	Work out travel routes.

They can support you to look after your health:

	Book and build confidence to go to important health appointments like:
	Annual Health Check Cancer screenings The dentist
	Find easy information to help you understand health conditions, like Easy Read or videos.
	Support you to meet your aims in your Health Action Plan.

Health Checks

Health checks are special appointments with a Doctor or Nurse each year.

You will be invited by your GP Practice every year, generally around your birthday.

It’s a chance for your Doctor or Nurse to get to know you and keep you healthy.

You can bring someone with you. We have guides and more information about how health checks help.

About your Health Check

If you feel like you would like to discuss support from our Social Prescribers, you can ask any member of staff at your GP practice to make a referral or use our online form.

Please note that our Social Prescribers do not prescribe any medication. They cannot complete forms or applications for you on your behalf. They are not support workers.
