These could be things like feeling socially isolated, having housing issues, financial problems, or mental health issues.
It is not only your GP who can support you to feel better.
Social Prescribers use a more holistic approach and can support patients with wide range of social, emotional, or practical needs.
How Social Prescribing Works
They will help you explore other things that you can access or do to help you feel better.
They will work with you to find out what issues are affecting your health and well-being, and support you to access services, groups, and resources in the community to improve your wellbeing and have more control over your own health.
Supporting you to become the best version of you.
Our team can support you with things like:
- Getting used to having a long-term condition.
- Feeling stressed or low.
- Getting advice on housing or benefits.
- Feeling lonely or isolated.
- Getting out and more active in the community.
- Support with your mental health.
- Getting involved in a new hobby or interest.

Getting in touch
If you feel like you would like to discuss support from our Social Prescribers, you can ask any member of staff at your GP practice to make a referral.
Please note that our Social Prescribers do not prescribe any medication. They cannot complete forms or applications for you on your behalf. They are not support workers.
