What I Do
I work with people who are on the learning disability register.
We can meet with people to find out what is important to them, identify goals and support them to reach these goals. We can meet regularly to support this. This may mean connecting other services, we can .
We can work flexibly depending on the needs of the person- first contact could be either in person, by letter or over the phone.
How I Support Patients
My role is to support people with a learning disability with social, emotional or practical needs that may be affecting their wellbeing.
This includes connecting people to other organisations where appropriate.
I have previously worked as a Social Prescriber and for Skills for People, who are a self-advocacy charity supporting people who have a learning disability and autistic people.
I have a degree in Health and Guidance & Counselling.
