Health and Wellbeing Services in Gosforth and Jesmond
If you’re a patient at a GP practice in Gosforth or Jesmond, you can now self-refer to see one of our Health and Wellbeing Coaches — no need to see a Doctor first.
This free service is part of the extra support offered to patients at GP practices by Gosforth and Jesmond Health, designed to help you stay healthy, active, and well.
How It Works
Health and Wellbeing Coaching is about practical, personalised support—not prescriptions.
Our coaches won’t give you medication or weight-loss drugs like Mounjaro. Instead, they’ll work with you to build lasting lifestyle changes that support your physical and mental wellbeing.
Here’s what to expect:
- You set the goal – whether that’s getting more active, eating better, losing weight, managing a long-term condition, or improving your mental health.
- We make a plan – your coach will help you understand your habits and create realistic steps you can stick to.
- You get support – your coach will guide, motivate, and support you to stay on track—without judgement.
Typically, we support people over 8 sessions, but that may vary from patient to patient, depending on need.
This is a non-medical service, focused on helping you take control of your health through education, encouragement, and expert coaching – not prescriptions.
Our Coaches
We are very fortunate to have two excellent Health and Wellbeing Coaches working with our GP practices in Gosforth and Jesmond.
Rachel McIntyre and Hannah Parks are both qualified health coaches and experts in nutrition. They offer support for weight management, living well with type-2 diabetes, and low-level mental health problems like anxiety.

Hello, I’m Rachel. I joined the GJH team as a Health & Wellbeing Coach in February 2025. I work with patients at all GP practices in Gosforth and Jesmond.
Hannah Parks
Health and Wellbeing Coach
I’m Hannah, and I joined Jesmond Lower Gosforth PCN in January 2024, having previously worked in North Gosforth since 2022. I work as a Health and Wellbeing Coach.
Getting started
We’ve put together some useful tools and resources you can start using straight away, so you can begin making positive changes even before your first session.

Our first recommendation is the NHS Weight Loss App. It’s free to download and use, and guides you through some simple changes to what you eat and drink.
